Authorization

I hereby authorize Assistify Services and its agents, including consumer reporting agencies such as GoodHire or Checkr, to obtain consumer reports and/or investigative consumer reports about me at any time during the hiring process and, if applicable, throughout my employment or contract period. I understand that I have rights under the FCRA, including the right to request and receive a copy of the report and to dispute any inaccuracies.

I certify that the information provided is true and correct. I acknowledge that this authorization remains in effect during the course of my employment unless revoked in writing.

Disclosure

In connection with your application for employment (including independent contractor or volunteer services), Assistify Services may obtain a consumer report and/or an investigative consumer report on you to evaluate your eligibility for employment, promotion, reassignment, or retention.

These reports may include information about your criminal history, motor vehicle records, employment history, education, credit standing (if applicable), and other background data.

This process is conducted in accordance with the Fair Credit Reporting Act (FCRA) and applicable state laws.
